Photo/Video Release and COVID Waiver Release of Liability Acknowledgements

By submitting a registration entry to Achievement Day this recital program I am agreeing to the following:

1) I grant permission to the Aurora Music Teachers Association (AMTA), on behalf of the child/entrant, for the use of photos taken during this event for the Association’s publications, website, Facebook page, and/or YouTube channel for the purpose of educational announcement.

2) I agree to indemnify, release or hold harmless Aurora Music Teachers Association, its agents or representatives, from any claims or causes of action arising or related in any respect to the event or the child’s/entrant’s participation in this event.

3) I hereby agree to release and hold harmless AMTA, its officers, directors, employees, and agents from any and all illnesses, injuries, sickness, death, or other losses arising in any way from attendee/registrant’s attendance at and participation in Achievement Day, including, but not limited to, exposure to COVID-19.
